Employee's fingers amputated when caught between rollers

Employee #1 was cleaning the moving roller of a metal forming mill line. He passed through a gap in an interlocked gate and climbed atop the mill platform to reach a section where debris was stuck to the roller. He took a rag in his gloved right hand and was trying to place it on the upward side of the running bottom roller when his hand was pulled into the nip point between the rollers. His index and middle fingers were crushed beyond reattachment before coworkers could stop the machine. Apparently, it was considered normal operating procedures for employees to clean the rollers while they were running.
