Employee fractured lumbar vertebrae in fall from roof

On March 15, 2001, Employee #1 and coworker were installing a plastic barrier on a 4-to-12 pitch roof, to protect the structure from a forecasted storm. Employee #1 stepped onto roofing felt that had been rolled out earlier in the shift. The felt had been rolled out onto a beam, giving a false impression of being on sheeting and causing Employee #1 to fall 8 ft to a roof ledge (4 in. by 16 in.). Subsequently he continued falling another 7 ft, hitting a garbage can, and he then fell to the ground. Employee #1 sustained serious injury due to the fall of 15 ft. Employee #1 fractured the lower lumbar vertebrae L 2, 3, and 4. Neither Employee #1 nor the coworker working on the roof was using a fall protection system.
