Investigation Summary

Investigation Nr: 171060627
Event: 02/28/2001
Employee crushed between gantry and table

Employee #1 was building roof trusses on a table located adjacent to a MiTek Roof Glider roof truss roller gantry. The gantry had just rolled over a truss being constructed and had stopped in the "parking zone". The gantry operator then reversed the gantry because it was covering a truss located on the manual production table, so the truss could be removed. After the gantry initially stopped in the "parking zone", Employee #1 crawled on the ground between two tables located on the side of the gantry where the gantry had just passed, apparently to retrieve some plates on the ground to use in constructing trusses. The gantry operator was unaware that Employee #1 had crawled in between the tables; when he reversed the gantry, Employee #1 was crushed between the outer gantry frame and a metal post of one of the tables. The operator realized something was in the pathway of the gantry but was unable to see Employee #1 until he exited the operator control platform and examined the area between the tables. Employee #1 was conscious and breathing for a short time (1-2 minutes) then became unconscious. Employee #1 entered cardiac arrest, and two employees immediately began CPR and continued until paramedics arrived a few minutes later. Employee #1 was pronounced dead in the hospital emergency room.
