Investigation Summary
Employee #1, a chemist, was burned on her face and neck by acid when the hoist came in contact with the container. The electrolysis process is where the copper plates are placed in several tanks to be coated. The copper plates are moved from tank to tank by hoists. The lab technicians check the various tanks at least once per shift. The sulfuric acid tank is checked and brought up to a concentration of 17, so that they can maintain an approximate concentration of 15. Sulfuric acid with a concentration of 96 is typically used. The technicians added between 0.25 and 3.5 gallons of sulfuric acid to the tanks when needed to reach the proper concentration. The technician was in the process of pouring acid into the tank. As the hoists are raised to move the plates, a rotating light on each hoist is activated. The technician stated that she misjudged the distance of the hoist. The hoist came into contact with the container, causing acid to spill on her face and neck. Employee #1 went to a local hospital for treatment.
