Investigation Summary
Two employees were painting a blue horizontal stripe on the side of a building. Employee #1 had just repositioned an extension ladder and was in the process of climbing it when he fell. His coworker was near the company truck getting new painting supplies and saw the accident happen. Employee #1 was hospitalized with lacerations.
