Employee fractured hip in jump from falling forklift

On January 30, 2001, Employee #1 was boarding a delivery truck equipped with a forklift and drove the lift over the dock plate. The truck rolled forward approximately 6-to-7 ft. The forklift fell between the truck and the dock landing on the rear counterweight. Employee #1 jumped off the forklift as it was falling and landed on the blacktop next to the forklift and broke his pelvis in two places when he landed. Employee #1 was hospitalized.
