Employee injured when he tripped and fell, breaking hip

Employee #1 was walking along in a warehouse where shelving was being removed. There were bolts sticking out from the floor. Employees were cutting or grinding off the exposed bolts. The employees had been informed of the bolts at a safety meeting minutes before the accident. The bolts had been marked with yellow paint. Employee #1 did not see the bolts, tripped on them, and fell on the concrete floor, breaking a hip. Employee #1 was hospitalized.
