Employee Injured In Deck Collapse

Four employees were working locking decking to beams at a height of approximately 20 ft above the fourth level. Approximately 900 sq ft of decking was in place but was not stable. A bundle containing 14 pieces of decking was placed on the completed but unstable deck. Three employees were about to secure the beams to the concrete columns when the shoring poles moved. Employee #1 had taken some of the decking from the bundle and dragged them across the deck. The moving the decking set up a harmonic motion to the unsecured decking. The foreman told Employee #1 to stop because the deck was shaking. The employees on the fourth level who were placing shoring poles warned that the poles were moving and told the employees working on the deck to get off before it collapsed. Before the collapse, the three coworkers were safely on a nearby column. One of the employees attempted to catch Employee #1 as the deck collapsed. The coworkers attempt to catch Employee #1 failed when a long beam caught on Employee #1's retractable line and striking him in the head. The beam also caused Employee #1's retractable line to extend to its full extent. All employees working on the decking were using full-body harness and 6-ft lanyards were attached to the lifelines above them. Employee #1 suffered bruised and abrasions but was not hospitalized.
