Employee's hand crushed when caught by spinning plate

On November 30, 2000, Employee #1, a trainee, was working his first day on this particular equipment. The regular trainer was on vacation. The supervisor stated that the normal procedure is to shut the equipment down during cleaning operations. However, an employee observed Employee #1 wiping the plate while the plate was operating or spinning. While wiping the plate, the plate caught the employee's hand, crushing it. Employee #1 was not informed of the proper procedures.
