Investigation Summary
On October 17, 2000, Employee #1 was engaged in line clearance tree trimming operations. The employee was wearing a saddle harness and a single rope lanyard. The employee untied his lanyard to reposition himself in the tree. When he untied, he lost his footing and fell 12 to 15 ft, landing on his right shoulder. He was hospitalized with a fracture.
