Investigation Summary
Employee #1, a machinery maintenance technician, was performing maintenance on the hopper of an iron ore skip car. The cause of the event was the emptying of 17 tons of iron ore from the scale car into the skip car. The ore entered so fast that Employee #1 did not have sufficient time to react to or escape from the onslaught of ore. The ore simply knocked him down and pushed him into the far corner of the skip car, where its weight crushed him to death.
