Investigation Summary
Employee #1 was using a table saw to cut through several layers of 1 in. thick sheets of styrofoam when it became caught in the saw. The styrofoam kicked back, pulling his left hand into the top, exposed portion of the blade. Employee #1 sustained severe lacerations to two fingers.
