Employee killed by fall, striking head on chain conveyor

At 4:30 p.m. on October 19, 2001, Employee #1 went to a chip mill to load wood chips onto his 18-wheel truck. No employees were on site that worked for the chip mill. For unknown reasons Employee #1 climbed onto the approximately 20-foot-high chip pile. At some point the pile gave way and Employee #1 fell, striking his head on part of the chain conveyor. The following morning when mill employees reported to work they noticed Employee #1's truck still running and the conveyor was on. The mill supervisor was notified and Employee #1's supervisor was called, arriving shortly at the chip mill. Employee #1's supervisor and others began to search for Employee #1. After the Sherriff's office arrived, the search continued and Employee #1 was located, buried in the chip pile. Employee #1 was pronounced dead at the scene.
