Employee caught by auger shafts

At approximately 3:00 a.m. on December 15, 2001, Employee #1 was draining a blood tank in the office area of Columbia Farms poultry processing plant. To drain the tank, he had to climb onto an elevated catwalk, then out onto a semi-trailer (office trailer) and insert the tank hose into the blood compartment area on top of the trailer. Two feathered augers are located in close proximity to the catwalk. One runs parallel with the catwalk (flat auger) while the other (incline auger) runs in an intersecting path to the catwalk. There were no witnesses observing Employee #1 performing this duty. Upon returning from break, another employee noticed blood coming from the offal area. The employee told another employee (truck driver) who climbed onto the catwalk where he found Employee #1 wrapped around the auger shaft that runs parallel with the catwalk. The employee who found Employee #1 then notified the shift supervisor who called an emergency rescue team. He died of his injuries.
