Event: 07/18/2001
One employee killed, second injured by degrouper machine

At 6:00 a.m. on July 18, 2001, Employee #1 was called to repair or replace an actuator arm on a limit switch to the degrouper machine. Employee #1 went in the hole under the degrouper machine in order to repair it, without locking out the machinery. As Employee #1 went into the area, the roll bumper activated and he was caught underneath. Employee #2 attempted to grab the machine arm to stop the cycle and his right hand was caught between the bumper and Employee #1. Employee #1 received fatal crushing injuries to his head and shoulders. Employee #2 received degloving trauma to the right fingers and hand for which he was treated and released.
