Investigation Summary
On June 6, 2002, Employee #1 was operating a Wheelabrator shot blast machine. A plant security guard discovered Employee #1's body lying by the machine's operating console. The coroner's preliminary findings indicated that the cause of death was positional asphyxia from falling, probably due to cardiac dysrhythmia. Employee #1 had a history of allergies and had visited his doctor earlier in the day for an allergy shot. A post-incident safety inspection performed by the employer did not indicate any problems with the machine.
