Investigation Summary
On April 1, 2000, Employee #1 was walking back from moving a tractor to a storage area and decided to hitch a ride. Employee #1 hitched a ride on a yard horse tractor and in the process of riding, fell off and was run over by the rear wheels of the yard horse. Employee #1 was taken to the hospital following the accident. On the next day, Employee #1 died as a result of the accident.
