Equipment Not Locked Out Shocks and Kills Worker

On December 12, 2001, Employee #1, a 39-yearold male, with Breitinger Company was working with two other coworkers and changing a 440-volt motor on a truss machine. The machine was not lockout and Employee #1 made contact with a live part and was shocked. Employee #1 was electrocuted and died as a result of this event.
