Employee dies from myocardial infarction

On June 12, 2001, Employee #1, a maintenance supervisor, was walking through a receiving area where employees were unloading large boxes with a forklift. He told the employees to use a crane instead of the forklift because the boxes were too tall, and the sling would not lift them. The supervisor then went over and started to pull on the sling, which weighted 197 pounds and was under a forklift attachment, which weighted another 291 pounds. He then pulled one more time and fell over backward. The supervisor had a history of heart problems and had just returned from sick leave as the result of a stroke. The supervisor had died of a myocardial infarction.
