Employee #1 died, neck caught between lapper and shaft
On February 7, 2001, Employee #1was working on a Garnett lapper machine in a confined space and the machine was not locked out. The machine over rode the limits and caught Employee #1's neck between the lapper and a drive shaft. He died as a result.
