Investigation Summary
On February 9, 2001, Employee #1, a construction worker, was standing on the top cover of a 42-foot-deep shaft while it was being moved with an industrial fork lift. The bottom cover accidentally rolled over causing Employee #1 to fall approximately 42 feet to the bottom of the shaft which contained about 8 feet of water. Employee #1 was killed.
