Employee Falls from Rack and Dies
An employee was trying to get a piece of plastic culvert pipe down from a rack about 4.5 feet off the ground for a customer. The employee, age 68, climbed up on the rack instead of using a forklift truck. The employee lost his balance and fell from the rack, striking his head on the ground.
