Investigation Summary
At approximately 1:00 p.m., an employee was cleaning the rolls of a tension stand, when the rollers pulled his gloves and both hands into the rollers. The employee had bypassed the control by sticking a razor blade to hold the jog control button down so the rollers could run continuously. The employee amputated three of his fingers at the second joint on his right hand and three of his fingers at the first joint on his left hand.
