Employee killed while cleaning machine

Employee #1, of Unilock, Inc., was wiping off a sensor device on a prelift machine. After cleaning the sensor, the machine, which was in automatic mode, cycled, pinning Employee #1's chest against the lowerator machine. He died of asphyxia caused by chest compression.
