Investigation Summary

Investigation Nr: 200351583
Event: 10/26/2000
Employee killed when struck by falling tree

At approximately 11:00 a.m. on October 26, 2000, Employee #1 and three coworkers were using a Long tractor, model 445, to fell a poplar tree with a forked trunk on a golf course. When they felled the right trunk of the tree, it became lodged in some trees to the northeast. They cut the pull rope they were using from the tractor and a coworker climbed a ladder and attached another rope approximately one-third up the remaining fork of the tree. He then walked under the lodged tree and up to the tractor, which was parked on a nearby road, and tied the other end of the rope to a hook on the backside of the front end loader attachment of the tractor. He and a second coworker were standing on the road as the tractor driver pulled forward and tightened the rope. Employee #1 then notched the northeast side of the tree and began cutting the opposite side. As the tree began to fall, Employee #1 escaped to the road and stood next to the tractor. The tree turned approximately 90 degrees, apparently due to the weight of its upper branches. The tension in the rope apparently pulled the bottom of the tree away from the road and the top of the tree fell toward the road where Employee #1 was standing. The first and second coworkers had already moved away from the tractor as the tree fell, and the driver crouched down in the seat of the tractor. The tree fell to the left side of the tractor, striking Employee #1 on the right shoulder and the head. Emergency medical technicians arrived at 11:25 a.m., but could not find any vital signs.
