Employee crushed in paper sorting machine

Employee #1 ran the sorting line that is part of a paper sorting machine. Employee #1 had started the "line" and then had gone to help dump carts of paper into the pit. Her coworker was running the front pit the line belt while Employee #1 was helping dump paper. Part of the coworker's job is to activate the leveler, which reduces the amount of paper on the belt that feeds the line. The line belt was activated a couple of times by the co-worker not knowing that Employee #1 was on top of the leveler, causing Employee #1's death.
