Employee Injured In Fall

Employees #1 and two coworkers were reassembling pipes and flanges in a room. The coworkers were working from a 3-ft unguarded scaffold. Employee #1 was working at the 12-ft level with his legs crossed between the frames and was reaching outside the frame to tighten the bolts on the flange. The flange was approximately 15 ft above the floor level. During the reassembling of the flange Employee #1 fell to the concrete floor suffering injuries to his head, left leg and left arm. Employee #1 was transported to the hospital where he was admitted and released the following day.Note: Injury Line on this report also lists two other hospitalizations for burns, but the narrative does not mention them.
