Employee Falls From An Unsecured Ladder

On September 27, 2001, Employee #1 and a coworker were installing Cooper Flashing Around Sky Lights on the north building. The pitch of the roof was approximately 12 to 12 and the deck to eve height was approximately 28 feet. They were using a 40-foot aluminum extension ladder. His coworker was installing the flashing, and Employee #1 cut and assisted with the installation of the flashing. His coworker had cut a length of flashing and had climbed to the roof's eave. When Employee #1 handed the flashing to his coworker, the flashing became lodged between the ladder and eave. Employee #1 pushed the top of the ladder from the eave to dislodge the flashing. When the top of the ladder was moved from the eave, the bottom of the ladder slid away from the building and off the deck. The ladder fell down to the deck, and Employee #1 fell down to the deck. Local police, Fire and EMS responded to the scene. Employee #1 was flown to PRMC by MSP helicopter. Employee #1 sustained a laceration to his head, fractures on his left arm, broken ribs and bruised internal organs. His employer failed to ensure that the ladder was secured at the bottom.
