Employee is killed when aerial work platform over turns

On May 5, 2000, Employee #1 and a coworker were operating a Grove AMZ131XT Aerial Work Platform. Employee #1 raised the platform to reach and clean the wire rope of a portal crane. The aerial lift was not tall enough and Employee #1 lowered the platform to the roof of the portal crane, 74 ft above the ground. The coworker stepped out of the platform. Employee #1 was proceeding to retract the boom of the aerial lift when the machine turned over due to a failure of the riser interlock system. This caused the boom and riser to be in an unbalanced position causing the machine to turn over. Employee #1 was whipped around by his fall arrest harness and died.
