Employee killed in excavation cave-in

Employee #1 and a coworker were on the second shift of a work crew using an air chisel and vacuum hose to remove fused ceramic media from the third canister of a regenerative thermal oxidizer. An approximately 4 ft long by 2 ft wide by 12 ft deep excavation had been created within the canister. Approximately 15 to 30 minutes after the second shift crew went into the excavation to continue the removal work, the east wall sloughed off, trapping Employee #1 against the west wall of the thermal oxidizer. He died of asphyxia due to chest compression.
