Fork truck rolls on to of Employee
Employee #1, a forklift driver, was unloading a truck when the driver of the truck drove away from the dock. The dock leveler that was resting on the truck was no longer being supported; it shifted, causing the fork truck which was atop it to lose control, fall, and roll over on Employee #1.
