Employee suffers seizure, killed in fall from scaffold

Employee #1 was on a tubular frame scaffold, handing forms up to a coworker when he apparently suffered a seizure. He landed on a scaffold outrigger, which was approximately 3 to 4 in. from the wall and about 12 in. below the scaffold frame. Employee #1 then rolled off of the outrigger to one side of the platform and fell approximately 59 ft between the scaffold framing onto the concrete floor. He was killed. Both the main and the outrigger were fully planked, and the platforms had perimeter guarding.
