Employee crushed and killed when marble slabs overturn

At approximately 8:15 a.m. on January 25, 2000, Employee #1, a helper, was selecting three marble slabs for a customer. The approximately 500 lb slabs were stored on an A-frame rack. To extract each slab, Employee #1 had to stand in front of the rack, tilt up a slab into a vertical position, and then attach it to the forklift with the use of tongs. As he was working, the slabs tipped over and crushed him against slabs on the adjacent A-frame rack. Employee #1 suffered serious head injuries, including a fractured skull, from which he died.
