Investigation Summary
On October 28, 2000, Employee #1 was driving a dozer over a hillside and was ejected from the dozer and struck his leg which was later amputated at the hospital. He died days later due to complications. At the time of the accident Employee #1 was suffering from a low blood sugar level and was attempting to get to a restaurant. Employee #1 was killed.
