Investigation Summary
At approximately 7:30 a.m. on November 9, 1999, Employee #1 was working on a two-story residential building. Employee #1 had "scabbed" a false tail onto a truss while standing on the top plate of a wall. Employee #1 also installed adjacent bird blocks. Employee #1 stepped on the false tail, which gave way, allowing him to fall approximately 17 ft striking his head on a sidewalk. Employee #1 was wearing a hard hat, and suffered no externally apparent wounds. Employee #1 attempted to stand up, then dropped to his knees, and finally fell face down. Employee #1 died from his injuries.
