Employee killed when crushed between conveyor and gate

Employee #1 was fatally crushed while working to free a log which had become trapped between the jack ladder conveyor and the diverter gate through which the logs were directed to the designated process areas. Employee #1 did not carry out the written procedures for lockout prior to entering the area.
