Employee killed when crushed by forklift mast

Employee #1, age 19, was sitting on a forklift vehicle with his right leg tucked under his buttocks. He raised up, apparently to get his leg out from under him, and fell forward, striking the DOWN lever. Employee #1's head became caught between the lowering mast and the frame of the forklift, and sustained crushing injuries. He was killed. Employee #1 had been operating this forklift for 10 months and had received formal training in July, 1999.
