Investigation Summary

Investigation Nr: 201260619
Event: 08/27/1999
Foot crushed by forklift and dies later from hemorrage

Employee #1 was driving a Dockstacker fork lift to move product in a freezer. He was heading west, down a well defined aisle way. Another employee was heading south, down an intersecting aisle way on an identical Dockstacker. At the intersection, the two vehicles collided. Employee #1 had his left foot crushed between the two vehicles. He was taken to the hospital where he had surgery to reconstruct his ankle the following day. The day after the surgery he went into a coma, and died the next day. The Cook County Medical Examiner attributed his death to a cerebella hemorrhage, and the manner of death was natural. The autopsy could not find any relationship between the injury and the cause of death.
