Employee Killed When Caught Between Mast and Cab

Employee #1 was working in the yard of the lumber company restacking materials. The left fork of the forklift became stuck in the side of an already stacked bundle of siding. Employee #1 stepped out of the cab and onto the right front fender of the forklift. He stepped between the unguarded cab and the mast. His foot slipped and contacted the mast control lever. The mast moved backward and trapped Employee #1 between the mast and the cab structure of the forklift. No other employees were in the area or witnessed the incident. Two other employees saw Employee #1 caught in the forklift and released the pressure from the mast. Employee #1 died while being transported to the hospital.
