Investigation Summary

Investigation Nr: 201260494
Event: 07/04/1999
Employee Loses Leg And Foot When Tractor Tipped Over

Employee #1 lost the lower part of his left leg and left foot when a tow tractor tipped over landing on his left leg. He was driving a tow tractor that he was not authorized to operate. He drove down a ramp that was 47 feet long and 15 feet wide, with an incline of 43 inches. Witnesses stated that he was driving "fast", and that he picked up speed as he went down the incline. As he attempted to make a right turn at the base of the ramp, the tractor started to lean to the left. Employee #1 attempted to stop the lean by sticking out his left leg. The tractor, however, continued to tip over, coming to a rest on its left side, with Employee #1's lower left leg and foot pinned under it. The injuries to the leg and foot were severe enough that amputation eight inches below the knee was required. He went to a local hospital for treatments.
