Employee killed in fall from scaffold

Employee #1 fell from a scaffold and was killed. He was not wearing any form of fall protection. Coworkers later stated that they had been informed that fall protection equipment was provided by Lile Construction for all contractors. Lile Construction also had safety meetings during which each contractor was instructed to wear fall protection when required.
