Investigation Summary
On May 24, 2000, Employee #1 was cutting and burning kiln rings near a pile of scrap steel. The pile of steel was approximately 127-in. by 135-in. by 38-in. He slipped, tripped, or fell off of the pile of steel and hit his head on another piece of steel measuring 34.5-in. by 12.5-in. by 13-in. high and died.
