Employee #1 was crushed to death

On February 6, 2001, Employee #1, Day Process Technician, for Tencel, Inc., was crushed to death by the mast of a forklift he was operating. Employee #1 had positioned his body between the lift's mast and overhead guard and he apparently attempted to adjust the tilt of the mast, using his right foot. The mast came back within seconds crushing him. Employee #1 died.
