Employee's arms and legs crushed by wheel rim

Employee #1 was welding on a 3,600 lb wheel rim that was set in a cradle and rollers so that he could turn the wheel by hand to complete his job. The wheel tipped over on him, crushing his arms and legs. Employee #1 sustained severe injuries, for which he was hospitalized.
