Employee dies when struck by a collapsing boom assembly

On January 21, 1999, Employee #1 was working on a skid steer loader to replace a broken directional arm for the motion control lever. The skid steer loader was manufactured by Hdra Mac, Inc., 12C Mitey Mac II , Serial Number 32612. He was used a wooden block or an aluminum pipe under the boom assembly area to support the bucket assembly. He did not place the boom and bucket control lever at the neutral position, but left the hydraulic system open. During his work, the wooden block or aluminum pipe became dislodged causing the boom to collapse. The collapse of the boom knocked Employee #1 out of the bucket and in between the boom assembly and the hydraulic equipment. The impact of the boom assembly upon him crushed his thoracic cavity and caused him to die from asphyxiation. The skid steer loader's engine was found not running (in the off position) and the key was not in the ignition switch. The boom and bucket control lever was found in forward position and the bucket was down and the motion control lever for direction was found in forward position too. There was no evidence of hydraulic oil spray indicating unintended opening of the hydraulic system.
