Accident Investigation Summary
Summary Nr: 200230878Event: 07/28/2000Employee Killed When Struck By Falling Screw Assembly
Employee #1 and coworkers were changing a large valve on the sixth floor of a pulp mill. The valve had an attached twin screw assembly that had to be removed before the valve could be changed. The crew detached the screw assembly and pulled it off the valve, at which point it was suspended by rigging from a trolley. The crew was pushing the assembly to the west when the trolley came off the I-beam to which it was attached. This caused one end of the assembly to fall, striking Employee #1 on the side of the head. He was killed.
