Employee killed when crushed by scissor lift

An employee entered the scissor lift pit on line number 2 to retrieve wooden stiles, which are used in the door manufacturing process. As he was removing the stiles, the main hydraulic hose line burst causing the lift to lose hydraulic fluid. As the fluid continued to escape, the lift began to descend slowly on top of the employee. The employee was crushed between the scissor lift table and the scissor lift arm mechanism killing him. The scissor lift was not braced to prevent the descent of the lift when the hydraulic hose line failed.
