Employee killed in fall when scissor lift overturns

Employee #1 was on a 14 ft high scissor lift, pulling new wires, when the lift was moved to gain more leverage. As the lift was backed up, the left rear wheel dropped off the 4 in. tall concrete slab. The lift tilted and fell, and Employee #1 was thrown to the ground. He sustained head injuries, from which he died.
