Investigation Summary
Employee #1 and a fork lift operator were preparing to ship out 20 ft long bundles of steel rods weighing approximately 900 pounds. While moving six bundles of rods with the forks of the truck elevated, Employee #1 walked under the load and was struck on the neck when one of the bundles fell. Employee #1 died.
