Employee killed when caught in mechanical power press

Employee #1 and coworkers were examining a 3,200 lb Hitachi Zosen mechanical power press on which the automatic die change had been initiated and malfunctioned. The front die had failed to enter the press, and Employee #1 was looking through the press from the back side. In testing the press, the operator inadvertently selected the rear die instead of the front one. Employee #1 was standing in the shear point of the rear die and became caught between the die bolster platform (the transfer rail) on the rear dies and the press frame. He was killed.
